This documentary, presented by Phil Gayle, is a moving story of four teenagers who grew up in care but are now leaving the system and making their own way in the big wide world.
The odds are stacked against care leavers; the statistics show that many of them get into trouble with the law, have children early or become homeless. Each of the young people featured have been matched with a volunteer mentor through projects set up and supported by the Prince's Trust.
 Mark and Brian The mentors are a bit older and take a special interest in the care leavers, meeting up with them for a few hours every fortnight. Leaving care is a time of transition and each young person faces their own personal challenge.
Brian has spent four years in care, and is getting his first flat. Like Brian, Mark, his mentor, is from south east London and wants to give something back from his own happy upbringing.
Carrie missed out on her education because of continous moving around care homes, and now with the help of Leslie, her mentor, a busy mum of four, is trying to redress the situation. Leslie provides Carrie with some time out and someone who is there to listen.
 Carol and Jane Jane is leaving her foster family, and is matched with Carol, who after finishing work felt that volunteering as a mentor was a perfect use of her free time. Carol encourages Jane to forge new friendships in her post-care world.
Daniel has been living with his foster parents since he was 2, and is preparing to leave care. Shy and withdrawn, he is matched with the outgoing Nathan, to build his confidence, and ease him into adult life.
